There’s a CHM interview from 2016 with Stu Wecker who was the original designer of DDCMP, looks like V3.0 was the first version that was released outside of DEC, based on the dates in the interview.

Looks like it did. There’s a conditional assembly parameter to control the version of DDCMP to use, so maybe V3.0 and V4.0.
